Case Management
The Model of case management is based on a person centered ‘abilities’ approach to helping you identify priorities in your life. Steps in achieving this are then recommended.
Case Management recommendations are based on the information provided by you, significant others. Requirements of the funding or referring agency will be considered. Assessments may need to be carried out in order to clearly understand you and your circumstances.
Our Code of Conduct and Ethics is of the highest standard and is consistent with NSW Health and the Case Management Society of Australia.
